Your funeral director will advise you if any specific information or items need to be brought to your arrangement conference. Some of the information or documents are used to prepare the death notice and death certificate. Some items are requested in the event that you have decided to provide a viewing and visitation.
The most commonly requested items of the deceased's are:
Full name
Date of birth
Place of birth (City/State or Province)
Social Security number
Mother’s first name and maiden name
Father’s full name
Marital status
Level of education
Usual occupation or career
Legal residence address
Service discharge document (DD-214) or United States Armed Forces serial number (if a veteran)
A recent photo of the deceased (for identification)
A portrait photograph that shows their hairstyle if there will be a private or public viewing
A photograph for the online obituary
Clothing, including undergarments
Dentures (if worn, ask your funeral director if recommended)
Eyeglasses (if applicable)
Jewelry (optional)
Pallbearers (a list of 6-8 people to carry the casket, if applicable)
